Dear Town of Phillipsburg,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

I am formally requesting access to all public records related to an incident that occurred on or about February 2, 2025, in the 160 block of South Main Street, Phillipsburg, NJ.

This request includes, but is not limited to, the following:

All incident reports, CAD call logs, and dispatch records connected to this event.

Any and all police body-worn camera and/or dash camera footage from responding officers.

Any 911 call recordings and/or dispatch audio and any corresponding transcripts.

Any arrest reports, citations, or field interview cards generated in connection with this incident.

Any supplemental documentation, including follow-up reports, internal memos, or classifications related to the event.

Any and all video recordings submitted to the department by civilian witnesses, including but not limited to those recorded on mobile devices or surveillance systems, in connection with the above incident.

I am specifically aware that a witness submitted a video recording to the department documenting the events that led to the initial call. This recording is an essential part of the public record and should be included in your response. If it has been uploaded, catalogued, or attached to a case or report, please provide a copy of the file or indicate how I may obtain access to it.

If any portion of the requested records is exempt from disclosure, I ask that you provide the non-exempt portions and cite the specific legal exemption for any redactions or denials.

Please provide these records in electronic format if available. If there are any associated costs exceeding $25, kindly notify me in advance.
