Dear Mr. Falvey:

Thank you for your July 17, 2026 response to Request No. 24655 and the enclosed Face Sheet and Progress Notes. Consistent with your letter, this is a new request for **specifically identified records** concerning me — Richard Toelk, SBI No. 000402870C, Inmate No. 1315999, in NJDOC custody approximately June 5 through December 22, 2023 (GYCF and SWSP). Each item below identifies a particular record reflected in the documents your office produced; none seeks research, compilation, or information in the abstract. *Bent v. Twp. of Stafford Police*, 381 N.J. Super. 30 (App. Div. 2005).

1. **The detainer packet.** The detainer logged against me on **August 14, 2023** (Order Type OS, Info No. **23080007**, "OUT-OF-STATE COURT," reflected on my Face Sheet under Detainers/Open Charges), together with the accompanying warrant, teletype, cover communication, and any documentation received from Warren County, New York or the New York State Police in connection with its lodging, as maintained in my classification or detainer file.

2. **The ISP cancellation record.** My Progress Notes record, on August 8, 2023, "Released to ISP 8/17/2023 ... no detainers" (case action "cat 2 - cleared for release"), and on August 14, 2023, "ISP COURT 8/17/2023 CANCELED." I request the referral, screening, eligibility determination, cancellation notice, or communication (including from or to the ISP program or the Administrative Office of the Courts) documenting that scheduled release and the reason for its cancellation.

3. **Incident and injury reports.** Any incident report, unusual incident report, Special Investigations Division referral or report, or injury report in which I am named as a participant, victim, or subject, for the period June 1, 2023 through December 22, 2023 — specifically including any report concerning an assault upon me.

4. **Special housing placements.** Any administrative segregation, protective custody, temporary close custody, or other special housing placement order or form concerning me during the same period, if any exist.

5. **Grievance log.** Your letter asks that I supply reference numbers for my inquiries and grievances. I do not have them; accordingly, I request the specific record that contains them: the electronic inquiry/grievance system log or index associated with Inmate No. 1315999 (listing reference number, date, and subject for each entry). Upon receipt I will identify any individual entries I wish to request.

6. **Release-coordination correspondence.** The communications between the Department and the New York State Police or Warren County authorities, during the period November 1, 2023 through December 22, 2023, concerning the detainer and my December 22, 2023 release — including the coordination reflected in my December 13, 2023 Progress Note ("CONFIRMED PICKUP WITH NY STATE POLICE FOR WARREN COUNTY, NY — INV. Attached to this email are the only 2 pages of records found pertaining to
the Intensive Supervision Program. On the 1^st page you will find a
"Cancelled" stamp on which is written "Per ISP". This is the only
information we have on file regarding this matter. More information on

3. **Incident and injury reports.** Any incident report, unusual incident
report, Special Investigations Division referral or report, or injury
report in which I am named as a participant, victim, or subject, for the

In response to this portion of your request, there is no indication in
your file of any type of incident occurring. Without further information
(i.e.: specific date, location, persons involved, etc.) we are unable to
determine if any records exist in any other unit in the Department.
4. **Special housing placements.** Any administrative segregation,
protective custody, temporary close custody, or other special housing
placement order or form concerning me during the same period, if any
exist.
In response to this portion of your request, your Progress Notes (provided
to you in previous OPRA Request #24655) list your housing assignments over
the course of your previous incarceration. There is no indication of any
special housing placements during your time in custody.
5. **Grievance log.** Your letter asks that I supply reference numbers for
my inquiries and grievances. I do not have them; accordingly, I request
