The Hazlet Township Municipal Clerk received your Open Public Records Act (OPRA) request on April 8, 2020. As such, the seven (7) business day deadline to respond to your request is April 20, 2020. This response to your request is being provided to you on April 13, 2020.

Attached you will find copies of Hazlet Township Police Department Use of Force forms from calendar January 1st, 2016 through December 31st, 2018.
