This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:
Any MVA crash reports, incident reports, case summary reports, and/or investigative reports (including any follow ups), with narratives, pertaining to the following incidents:
1. Motor vehicle incident on Cedar Bridge Road on 3/30/82 involving R. Gant.
2. Incident at 964 Rosewood Avenue on 10/14/85 involving M. Ram.
3. Incidents at multiple locations at 300 Brick Boulevard and also Chambers Bridge Road on 3/19/87 involving L. Rosa.
4. Incidents at multiple locations at 459 Jackson Avenue and also Rosewood Avenue on 7/17/92 involving J. Radomski, L. Radomski and J. Grant.

N.J.S.A. 47:1A-1 The following records shall not be considered to be government records subject to public access : Information concerning individuals relating to medical, psychiatric or psychological history, diagnosis, treatment or evaluation.

N.J.S.A. 47:1A-1.1 Criminal investigatory records - records which are not required by law to be made, maintained or kept on file that are held by a law enforcement agency which pertain to any criminal investigation or related civil enforcement proceeding. (Note: N.J.S.A. 47:1A-3.b. lists specific criminal investigatory information which must be disclosed).
4. Exempt citing:
N.J.S.A. 2A:4A-60 Social, medical, psychological, legal and other records of the court and probation division and records of law enforcement agencies, pertaining to juveniles charged as a delinquent or found to be part of a juvenile-family crisis, shall be strictly safeguarded.
