Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

I am writing to inquire whether your office may be able to locate and provide a copy of a police or investigative report related to a shooting incident that occurred in Union City, New Jersey.

The incident took place on February 14, 1996 (Valentine’s Day) inside a nightclub called Las Vegas, which was formerly located on 5th Street in Union City (the establishment no longer exists). I was a victim in this incident and sustained a gunshot injury to my mouth from a stray bullet while I was exiting the restroom. I was subsequently transported to Jersey City Medical Center for treatment.

To the best of my recollection, the shooting occurred during a dispute involving individuals believed to be affiliated with the MS-13

I initially contacted the Union City Police Department to request a copy of the report; however, I was informed that due to the age of the case, their department no longer maintains the records. I was advised to contact the Hudson County Prosecutor’s Office and to submit my request via email.

I am requesting this report for immigration purposes, that is I need an official record that I was physically present in the country on the date that the incident.
