Dear Newark City,

This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.

I am a SIU Investigator with Liberty Mutual.

We were advised that on 7-18-2019, Newark Police were called to 215 Fabyan Place, for an incident involving sulfuric acid that had been spilled onto multiple individuals. Emergency Services were reportedly called to the scene.

I am seeking the police report associated with this incident. (I do not have a case number).

The Liberty Mutual claim number is 040730590.

I am respectfully requesting this report for purposes of the SIU Investigation.
