1). I request a copy of Donna Messina's completed questionnaire provided to the Division of Local Government Services
pertaining to her recent (June 2018) reappointment as Alpha's Acting Municipal Clerk (NJSA 40A: 9-133c). Include the explanations that were submitted with the questionnaire, if applicable.
2). I request a copy of the letter requesting her reappointment that accompanied the above requested questionnaire.
