Dear Nancy Ippolito,
I'm not legally required to fill out your form and that my email request is enough. Please comply with my OPRA request within the legal time period of 7 business days. Pursuant to Renna v. County of Union (App. Div. 2009), OPRA requests should be on the agency’s official OPRA request form. However, a written request that is not on the agency’s official OPRA request form cannot be denied solely because the request is not on the request form. Written requests which are not on the form must mention OPRA. Therefore, if a requestor elects not to use an agency’s official OPRA request form, he/she must submit an otherwise written request that clearly identifies itself as an OPRA request.

NJ Media:
As per your request of Nov 21, 2017 for all Opra files received between
August 1, 2017 and November 20, 2017 at the office of the MCRHC.
There are 27 Opra’s on file for that time period. Due to the record
volume, a NJ Media representative would need to schedule a visit to the
MCRHC office to review each Opra. Should you wish to copy any / all of
these Opra’s, a fee will be assessed at the time of visit. The MCRHC
office is open Monday – Friday 8:30 – 4:30pm. Please be sure to schedule
a visit to ensure the Opra Custodian is available to assist you.

As per your 11/21/17 Opra request (coded 11-07), attached are the MCRHC logs and actual Opra requests received during the months indicated on your request. They are coded by MM-##, i.e., 11-07 is 7th Opra in Nov. I have redacted the telephone numbers of the requestor and in one instance, the name of the person associated with medical records requested.
As a reminder, the Open Public Records Act allows for a fee to cover our copying or mailing. The fees do not apply in this instance, however, if future information is requested, applicable charges may apply. No charges apply when using email or fax.
