Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

This request includes, but is not limited to, the following: Police Dept Records; All incident reports, investigation reports, & supplemental reports, Names & badge numbers of all officers present on scene, Dispatch records, inc CAD reports & call logs, All radio transmissions / 2-way communications (audio & transcripts if available), Body-worn camera footage from all responding officers, Dash camera footage from all responding vehicles, internal notes, narratives, or supervisor reports, use-of-force reports, complaints, follow-ups, or internal reviews related to this incident. Communications: All emails, phone logs, recorded calls related to the welfare check,
communications bw the police dept & Citta Elementary, Any communications bw officers, dispatch, or third parties regarding this incident
School Records (Citta Elementary School):
Identity (name & title) of the individual whom initiated this welfare check for Anthony Guzman Jr., all reports, notes, or documentation that led to the request, All emails, internal communications, & external communications related to the request, Any calls made to/from school re this matter (including logs or recordings if available) I am requesting complete, unredacted copies of all records unless specific redactions are required by law. If any portion of this request is denied, please provide the legal basis for each denial as required under OPRA. If records are avail in digital format, I request they be provided electronically via email. This request is part of an effort to obtain a full & accurate record of the events of that day.

I am writing to request an internal review of Toms River Police Department's handling of my OPRA request 'OPRA Request for Records – Welfare Check on 11/23/2025'.

I've received no reply or response for my request for the body work camera footage of officers unlawfully entering my home with guns drawn pushing my kids out the way to gain illegal entry . It's been over a month and I would like an update ASAP kindly. Thanks in advanc

Dear Ewen, Heather J.,

Please tell me how I can proceed if the juveniles belongs to me then where and how can I privately request and receive such records. I have already sent a complaint regarding this incident and all officers included with the ocean county prosecutors office as well as internal affairs and as of right now the investigation is active. I still need a copy of records so please tell me how I can do so if not via here because of juveniles as you know your reply didn't provide alternative ways to receive such info. You can redact their names ofc but let me know how I should proceed please. This would've been extreme helpful information in your initial reply ....
