OPRA request for MVA/Pedestrian 10.19.2020 Lyndhurst, NJ
Please be advised that I represent David Segro , for injuries resulting from a motor vehicle/pedestrian accident that occurred on Oct-19-2020, case number 20-13291.
Please provide the following:
(1) A complete copy of the entire investigation file regarding this incident;
(2) All surveillance video and/or traffic camera video at Pennsylvania Ave (150 ft east) of Orient Way.
(3) All police dash camera videos including the audio recordings related to this incident;
(4) All police body camera videos including the audio recordings related to this incident;
(5) All notes taken by the responding police officers including the name, address, and telephone numbers of all witnesses to this incident;
(6) All photographs and/or videos regarding this incident;
(7) All reports from the Police Department, Fire Department and EMS regarding this incident;
(8) All CAD reports regarding this incident;
(9) All 911 phone call audio recordings describing and/or mentioning this incident to the 911 dispatcher, police, and/or fire department.

With regard to your OPRA request dated October 29, 2020, please see
attached responsive documents/reports.
As per your requested items (s) 1, 2, and 3, the Township does not
maintain any document responsive to this request, and therefore none is
being provided.
Regarding item #4 (BWC videos) are available for your pick up at our
office. There are two discs responsive to this request. There is a charge
of $1.00 for each disc, due upon receipt.
Regarding item #5, the Township does not maintain any document responsive
to this request, and therefore none is being provided.
Regarding item #6 (photographs), the Township does not maintain any
document responsive to this request, and therefore none is being provided.
Regarding item #7, any Police reports are attached. The Lyndhurst Fire
Department did not respond to this incident, therefore the Township does
not maintain any documents responsive to this request. Any request for
Ambulance or EMS/EMT records must be submitted to them directly via HIPPA

Regarding item #8, any Police CAD reports are attached.
Regarding item #9, please note that there is one (1) disc available
containing 911/Audio call relating to this incident. There is a charge of
$1.00 for this disc, due upon receipt.
The three (3) discs responsive to your request are available for pick up
at our office. Payment of $3.00 would be due at the time of receipt. If
you would like these discs mailed to you, please advise. We will require
payment of $3.00 plus the cost of postage prior to mailing.
