Dear Records Custodian,

Pursuant to the New Jersey Open Public Records Act (OPRA), N.J.S.A. 47:1A-1 et seq., I am requesting copies of all available video footage related to a motor vehicle accident that occurred on April 2, 2025, at approximately 8:30 AM at 23 Clover St, Lakewood, NJ. The incident involved a car striking an ambulance.

To facilitate the processing of this request, please provide:

1. Body-worn camera footage from all officers who responded to the scene.

2. Dashboard camera footage from any responding police vehicles.

3. Surveillance footage, if available, from nearby cameras.

I am willing to pick up the records in person if needed. If there are any fees associated with the processing of this request, please inform me in advance. Should any portion of this request be denied or redacted, please provide a detailed explanation citing the specific legal basis for the denial.

Please confirm receipt of this request and provide an estimated timeline for fulfillment.

As per the new
statute signed by Governor Murphy for OPRA,  the State now requires us as of 9/3/2024, that if you are submitting at OPRA via email or you are utilizing an old form that you need to certify the answers to the questions included on their State’s new OPRA form.
