OPRA Request for Bodycam footage
Dear Plainfield Police Department,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM seeking records in connection with a legal proceeding.
Records requested:
We present Citizens United Reciprocal Exchange ("CURE") with regard to a motor vehicle accident. On September 1, 2025 at approximately 4:03 a.m., our insured Felix Guadalupe-Menor's vehicle was involved in a motor vehicle accident (Case No.: 25033282). According to the enclosed police report, the reporting officer was Officer, Cameron Smith, Badge # 0304.
CURE respectfully requests that you maintain said Body Worn Camera video/surveillance footage and that same be provided to CURE. Please advise if there is any cost associated with obtaining this video. Failure to preserve this evidence may result in a court action seeking damages.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

vehicle crash / DWI investigation. Under OPRA, the videos will require

Please be advised, our office does not represent the driver in the accident. We represent Citizens United Reciprocal Exchange ("CURE"). The insurance company for the owner of the vehicle. They are conducting an investigation and are requesting the bodycam footage.

to send out. To view the Body Worn Camera (BWC) video, open the email and
follow the link. Audio will begin after the first 30 seconds. You may view
the video for up to one year from the date received. Please be advised
that unlisted telephone numbers, driver’s license numbers, CJIS data,
information related to medical treatment, diagnosis, or evaluation, are
exempt from OPRA and have been redacted from the police report as well as
the BWC video. Should you have any additional question or concerns
