Please accept this electronic request for public records made under OPRA and the common law right of access. I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

Records requested:

All public records related to Case #24-23983, a DWI arrest that occurred on June 14, 2024 at approximately 2:14 AM near W. South Avenue & Cumberland Street, Westfield, NJ, involving arrestee Crystal Beverly (35, of Elizabeth).

Specifically, I am requesting:
1. The complete police incident, arrest, and supplemental/narrative reports.
2. All body-worn camera (BWC) and patrol vehicle dash-camera video/audio recordings from responding officers (including Sgt. Gill, Officer Martinez, Officer Marlow, Officer Picciano, Officer Barham, and Officer Donovan).
3. Any field sobriety test results, breathalyzer/BAC test documentation, and DWI processing forms.
4. Any CAD call logs, radio dispatch recordings, and 911 call recordings related to this incident.
5. Any booking photographs, evidence/property logs, or chain-of-custody documentation connected to this arrest.
