Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

All records related to the November 9, 2023 incident in which Bradley Beach Police Chief Leonard Guida arrived at a DWI crash scene in an allegedly intoxicated state and was physically restrained by officers including being slammed onto a car hood. Also requesting all records related to the Monmouth County Prosecutor's Office investigation and report into Chief Guida's misconduct, including: all body camera footage from all officers present at the November 9, 2023 incident, all dashcam footage, the full Monmouth County Prosecutor's Office misconduct report issued on March 1, 2024, all internal affairs investigation records relating to Chief Guida, all records related to separate incidents in which Guida allegedly spat on an employee, shoved an employee into a chair, used profane language toward staff, and engaged in racial profiling, all records related to Guida's placement on paid administrative leave, and all records related to his retirement effective March 1, 2024.
