REQUEST 1 — EXTERIOR SURVEILLANCE CAMERA FOOTAGE — NEWARK POLICE DIVISION 5TH PRECINCT
All exterior surveillance camera footage from the Newark Police Division 5th Precinct located at 480 Clinton Avenue, Newark, New Jersey 07108, recorded on August 9, 2026 between 11:45 AM and 1:45 PM. This footage will show the arrival and extended presence of an individual at the precinct beginning at approximately 11:55 AM in connection with a court-ordered parenting time exchange scheduled for 12:00 PM at that location.

REQUEST 2 — BODY CAMERA FOOTAGE — OFFICERS CLARK AND MUGAVERO
All body camera footage recorded by Newark Police Division Officer Clark and Officer Mugavero, Unit 415 Tango, 4th Precinct, on August 9, 2026 in connection with Event Number P26419806, including all footage recorded during the police escort to 814 South 13th Street, Newark, New Jersey 07108 and all footage recorded at that location during the escort. This footage was recorded at approximately 1:30 PM on August 9, 2026.

REQUEST 3 — DASHBOARD CAMERA FOOTAGE — UNIT 415 TANGO
All dashboard camera footage recorded by Unit 415 Tango on August 9, 2026 in connection with Event Number P26419806, including footage recorded during transit to 814 South 13th Street, Newark, New Jersey 07108 and while at that location.

REQUEST 4 — COMPLETE CAD REPORT AND INCIDENT REPORT — EVENT NUMBER P26419806
The complete Computer Aided Dispatch record for Event Number P26419806 dated August 9, 2026, including the full call narrative, dispatch time, arrival time, officer assignment, unit assignment, call notes, all updates entered during the call, and the disposition. Also any incident report, supplemental report, or related documentation generated by Officers Clark and Mugavero or any other officer in connection with Event Number P26419806.

REQUEST 5 — COMMUNICATIONS AND NOTES RELATED TO EVENT NUMBER P26419806
Any radio communications, dispatcher notes, supervisor notifications, chain of command communications, or written notes generated in connection with Event Number P26419806 on August 9, 2026, including any communications between Officers Clark and Mugavero and their supervisors or chain of command regarding the family court order presented during the escort at 814 South 13th Street, Newark, New Jersey 07108.

REQUEST 6 — CHAIN OF COMMAND COMMUNICATIONS — EVENT NUMBER P26419806
All records of any telephone calls, radio transmissions, text messages, emails, CAD notes, or any other communications made by Officers Clark and Mugavero or any Newark Police Division dispatcher on August 9, 2026 in connection with Event Number P26419806 in which officers contacted their chain of command — including any sergeant, lieutenant, captain, or superior officer — to seek guidance regarding a family court parenting time order presented during the escort at 814 South 13th Street, Newark, New Jersey 07108.

This specifically includes:

— The identity of the superior officer or officers contacted in the chain of command call
— The time of the chain of command call
— The substance of any guidance or instruction given to Officers Clark and Mugavero regarding the family court order
— Any CAD notes, supplemental entries, or written documentation reflecting the outcome of the chain of command call
— Any recording of the chain of command call if captured through radio, body camera audio, or any other recording system
— The chain of command's response after being advised that the 30-minute waiting period provision in the court order does not take effect until September 4, 2026

PRESERVATION DEMAND
I formally demand that all of the above records be immediately preserved and not deleted, overwritten, or modified pending the processing of this request and any subsequent legal proceedings. This preservation demand applies specifically to:

— Dashboard camera footage from Unit 415 Tango for August 9, 2026
— All CAD records for Event Number P26419806

— All radio communications and chain of command communications for Event Number P26419806
— All recordings, notes, and communications from the chain of command call made by Officers Clark and Mugavero during Event Number P26419806

Body camera and surveillance footage subject to automatic deletion policies must be placed on a legal hold immediately upon receipt of this request. This matter is the subject of active family court proceedings in Essex County Superior Court, Docket No. FD-07-002853-23, with trial scheduled for December 17, 2026.

If any portion of this request is denied please specify the legal basis for each denial individually so that I may exercise my appeal rights under OPRA and before the Government Records Council.
