Please be advised that this office represents the Buyers in connection
with the purchase of the above referenced property.
Request the following certification:
1.      copies of all OPEN and CLOSED permits, certificates of approval
and approvals for any improvements made on the Property.
2.      records of any violations on the property
3.      records of oil tanks (underground or aboveground), and/or records
of gas conversion
4.      records of septic tanks, and/or pubic sewer connection"

Please find attached The "Construction Permit Summary Report" from our
current building permit tracking software SDL which shows only the permits
listed under the current block and lot number. The "List of Applications"
is from our former software, Mitchell Humphreys Construction Project
Manager and shows the permits from 1997-June 2013. Any permits that were
closed out after June 2013 will be closed in SDL and will show up on this
report as open. The third report is the "Extract of permits" which is a
listing of all building permits by street from 1946-1996. In this case,
the permits for 40 Brookside Ave have been highlighted.These 3 reports
will show if a permit was issued for the installation, removal or
remediation of an oil tank. There are no certificates of approval on file
for this address. The property owner(s) can contact the building
department at (973) 535-7957 or email [email address] to get the
status of any open permits.

The Zoning Official has confirmed that there are no open code violations
for the requested address. 
The Tax Department has confirmed that the requested address is connected
to the public sewer system. 
As these documents are being emailed to you there is no cost for
duplication under the Open Public Records Act.
