Dear Waldwick School District,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Pursuant to the New Jersey Open Public Records Act, I am requesting records concerning the artificial turf at Waldwick HS Stadium Field and Waldwick HS Turf/North Field, 155 Wyckoff Avenue.

For January 1, 2019 through the present, please provide:

1. All GMAX/G-Max testing reports, results, certificates, raw test data, individual test-location measurements, maps or grids, findings, recommendations, and pass/fail determinations for both artificial turf fields.

2. All proposals, quotes, invoices, purchase orders, contracts, and payment records for GMAX testing of either field, including records identifying the vendor or individual that performed each test.

3. All maintenance, repair, or remediation records associated with GMAX results or field hardness, including grooming, brushing, decompaction, infill replacement or redistribution, cleaning, and other work intended to affect shock absorption or surface hardness.

4. Records documenting any GMAX failure, exceedance of a testing threshold, retest, field closure, use restriction, or other action resulting from GMAX testing or concerns regarding surface hardness.

5. Records identifying the GMAX testing standard, specification, threshold, or pass/fail criteria used for each test.

6. Records identifying the manufacturer and installer of the current artificial turf system at each field, installation or replacement date, product/turf system installed, expected useful life, warranty, and any warranty claims concerning the playing surface.
