OPRA Report Request for 49 Stratford Dr Brick NJ 08724
Dear Brick Township,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

Owner Michael Shaw.
1. A list of all closed permits affecting the property, if any
2. Any and all open permits affecting the property, if any
3. A copy of any certificates of occupancy affecting the property, if any
4. A list of any open violations affecting the property, if any
5. a copy of all plans, documents, permits and other records concerning the recent renovation, if any
6. A copy of any surveys on file pertaining to the property.
5. Documentation related to any oil, septic or other underground storage tanks installed, abandoned or removed on the property if applicable.
