Hi Nicole,
Please access the portal using the following:
https://middletownnj.nextrequest.com/
Username: [email address]
Password: NextAccess99!
Then click on the top link All Requests. This will show you all your requests, open a closed request and you can view the .pdf documents on the left. The two most recent requests have not been closed yet, you will receive an email once the records are available and you can login. You can change your password through this system if you wish.
