Daniel R. Keene

Dear Montville Township,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Pursuant to N.J.S.A. 47:1A-1 et seq., I request access to the following records from the Montville Township Police Department for the period January 1, 2013 through December 31, 2013:
1. Computer-aided dispatch (CAD) logs or summary-level dispatch records that reference or involve Jason A. Condit, who is believed to have resided in Montville during this time.
2. Incident reports created during that period that reference or involve Jason Condit.

This request is limited to summary-level administrative records, including:
• Date and time of incident,
• Nature or type of incident (e.g., disturbance, weapon call),
• General location,
• Disposition or outcome,
• Any reference or incident number.

This request does not seek narrative investigatory reports or victim-related records and excludes any material exempt under N.J.S.A. 47:1A-3. If any record contains exempt material, I request that the non-exempt portions be provided with redactions pursuant to N.J.S.A. 47:1A-5. If no responsive records exist, I request written confirmation.

My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

Yours faithfully,

Daniel R. Keene

Dear Police Records,

I would like to clarify that my original request referred to Jason Condit, and not necessarily “Jason A. Condit.” If your search was limited strictly to entries containing the middle initial “A.,” I respectfully ask that you also search using the name Jason Condit only, without a middle initial, to ensure comprehensive results.

All other aspects of the request remain unchanged. Please let me know if any responsive records are located based on this clarification.

Yours sincerely,

Daniel R. Keene
