Please accept this email as an official request for public records under the Open Public Records Act (OPRA) and the common law right of access. Pursuant to N.J.S.A. 47:1A-6(f), I am not required to complete a specific form or use a designated software platform to submit my request. An email containing all information required by the agency’s adopted form is sufficient to constitute a valid government records request.

I WILL NOT use the requested government records for a commercial purpose.

- Property Survey
- Zoning applications/approvals
- Proof of decommissioned oil tank, DFT letter
- septic plans, as-built, repairs, septic reports and survey; and, if applicable, a septic survey showing the location of the decommissioned/abandoned septic tank, cesspool, and/or seepage pit.
