Records requested:

The Covid-19 date of diagnosis from March 5, 2020, to May 22, 2020, regarding the "Manager Checklist For Covid-19 Confirmed Case" and all documents regarding Covid-19 from the business located at 320 Route 17 South, Mahwah, NJ 07430.

Lastly, I was informed by multiple parties located in Bergen County that I would have to obtain these records from Northwest Bergen Regional Health Commission through the Mahwah Township.
