Dear Waldwick Borough,

I’m requesting documents concerning the Covid-19 date of diagnosis from March 22, 2020, to May 22, 2020, regarding the "Manager Checklist For Covid-19 Confirmed Case,” from the business located at 320 Route 17 South Mahwah, NJ 07430.

I learned the records I'm requesting exist from a FOIA request and Northwest Bergen Regional Health Commission located in Waldwick, NJ received these records. Also, Record Retention for Record series# 0156-0000 regarding Contagion Records is stored for two years.

Lastly, the municipality of Mahwah in writing stated to me on 2/3/2022 that they do not have the records I'm requesting.
