Dear Bergen County,

I’m requesting documents concerning the Covid-19 date of diagnosis from March 15, 2020, to May 15, 2020, regarding the "Manager Checklist For Covid-19 Confirmed Case,” from the business located at 320 Route 17 South Mahwah, NJ 07430.

Northwest Bergen Regional Health Commission has these records and the REGISTRAR OF VITAL STATISTICS refused to state to me by phone and e-mail if the Health Commission still holds these records.

I learned the records I'm requesting exist from a FOIA request. Also, Record Retention for Record series# 0156-0000 regarding Contagion Records is stored for two years.
