Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

Pursuant to the New Jersey Open Public Records Act (OPRA), I am requesting copies of all records maintained by Little Egg Harbor Township relating to the property located at:

Including any records associated with the above address and/or corresponding block and lot number.

Please include, but not be limited to, the following:

• Zoning designation for the property and any zoning compliance records
• Building permits issued for the property (open and closed)
• Certificates of Occupancy and/or Certificates of Continued Occupancy
• Building, zoning, and fire inspection records, including final approvals
• Notices of Violation, code enforcement actions, or property maintenance complaints
• Records of any open or unresolved violations
• Planning Board or Zoning Board applications, resolutions, or approvals related to the property
• Flood zone determination, elevation certificates, and floodplain development permits (if applicable)
• Septic system and/or well permits and approvals (if applicable)
• Environmental or health-related records associated with the property
• Current tax assessment and tax status records

This request is made in connection with a residential real estate transaction. If any portion of this request is unclear or overly broad, please advise so that it may be clarified.
