Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

Please be advised that this firm has been retained by Citizens United Reciprocal Exchange (CURE) with regard to a motor vehicle accident which occurred on May 29, 2024 at approximately 7:14 pm. Wherein our insured’s vehicle was involved in a motor vehicle accident with a pedestrian (Case No.: C24045187). According to the police report (which can be provided upon request), the reporting officer was Police Officer, Christopher Green,
Badge #11553. CURE respectfully requests that you maintain the camera footage from the scene of the accident and that same be provided to CURE.

Please advise if there is any cost associated with obtaining this video. Failure
to preserve this evidence may result in a court action seeking damages.
