This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
I am requesting the following records:
To the fullest extent permitted under law, please provide (via email) the following for each person currently employed by the Newark City Department of Health and Community Wellness: (1). The individual’s name, title, position, salary, payroll record, length of service, date of separation and the reason therefor, and the amount and type of any pension received or to be received; (2). The individual's disciplinary record, including record of any suspensions, corrective action, civil service hearing, appeal, or settlement received by each person currently employed by the Department.
