- It can be chart or a directory that clearly shows the name, title and direct reports to each person who is in charge of the the New Jersey Department of Health NJ COVID Community Corps
- There should be email and phone numbers associated with this department.
- Total number of employees working for this department.
- Confirmation from the New Jersey State Police that criminal background checks have been conducted on all members having contact with the community.

- This can be in a table format and can be listed by date, time, county, city/municipality
All of the files can be in PDF or whatever electronic media is easiest to retrieve and send to me. There is no need for any paper.
I understand this could take some time, I would really appreciate to have the listing of all locations visited and planned to visit first, if possible.

I am requesting the titles of any and all groups that are assisting or acting as employees or volunteers for the New Jersey Department of Health NJ COVID Community Corps. This includes any political organizations, any trade unions, civic groups; etc.
I also need the complete names and titles of any all personnel from said groups/organizations that are taking part in this effort.

The public has a right to know:
* Who are the people being sent into neighborhoods with personal HIPPA information about the residents of said neighborhoods?
* How and when were criminal background checks performed on the employees who are handling this personal HIPPA information?
* Who is running this department: "COVID Community Corps" and who are the people reporting to those in charge?
* Who is running the COVID Community Corps Facebook page?
* Where is the schedule showing where and when these unnamed/unknown people are being deployed?
The lack of transparency of this group of unnamed people is disturbing to say the least. The public has no idea who these people are and they have a right to now immediately. You are giving me the impression that local Health Departments and Police Departments know who these people are and when they are going to be asking private citizens about personal HIPPA information.
A complaint will be filed with the records board if this matter is not resolved immediately. I have legal counsel at the ready to escalate this matter if necessary.

Please pass this on to the person who conducts Open Public Records Act reviews.
I am writing to request an internal review of New Jersey Department of Health, Division of Epidemiology, Environmental, and Occupational Health's handling of my OPRA request 'New Jersey Department of Health NJ COVID Community Corps Information'.
I am finding it hard to believe that the department cannot tell me:
1) Who is running the department
2) Who works for whom (organizational chart)
3) The source of the canvass workers: are they from a political group, an employment agency, are they answering "help wanted" ads?
4) How are the canvass workers screened? Meaning, did they get a criminal background check? If they did not, how are they allowed to work for the government and handle HIPPA information?
5) Where are the records that show, state-wide: where did they canvass in the past, and where are they going in the future?
I do not understand why such basic information is so difficult to answer. It is not creating any confidence in the department and how it is handling extremely sensitive information.
My attorneys are growing impatient and I so am I -- please, can someone produce this information?

This is totally unacceptable. How can this department receive millions in tax dollars and is supposed to guard the public’s health, but:
* Cannot produce ANY documentation showing “who does what”
* Cannot make a statement verifying the “COVID Corps” has been vetted via a criminal background check?
* Cannot explain HOW you obtained the information, who is handling the information and who oversees the operations in your department?
