Dear Asbury Park City,
Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.
I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.
I WILL NOT use the requested government records for a commercial purpose.
I AM NOT seeking records in connection with a legal proceeding.
Records requested:
- The report, study, management review, operational review, fiscal review, staffing study or other assessment prepared or issued in or around 2005 by the New Jersey Department of Community Affairs, Division of Local Government Services, concerning the City of Asbury Park Fire Department.
- All appendices, exhibits, staffing tables, organizational charts, financial analyses, recommendations, transmittal letters and final supporting materials accompanying that report.
- Records identifying the date the report was received by the City and the City officials, departments, governing-body members or employees to whom the final report was distributed.
- City Council resolutions, meeting minutes, agenda packets, memoranda, final implementation plans, final corrective-action plans or final written responses addressing the report or its recommendations.
- Final records identifying which recommendations were accepted, implemented, rejected, deferred or left incomplete.
- Final organizational charts, tables of organization, staffing plans or budget documents created in direct response to the report.
- Any later final report, memorandum or presentation, dated January 1, 2005 through present, that compares the Fire Department’s subsequent staffing or operations with the findings or recommendations of the 2005 Division of Local Government Services report.
This request refers to an identifiable report believed to have been prepared in or around 2005 concerning the Asbury Park Fire Department. It does not require the custodian to research or determine the answer to a question. If the report is maintained under a different title, please produce the responsive record based upon the issuing agency, approximate year and subject matter identified above.
If no responsive copy is maintained, please provide any existing records-retention or disposition record showing when, pursuant to what retention schedule and under whose authorization the record was destroyed or otherwise disposed of.
