Dear Mr. Harley,
Thanks for the responsive records. I noticed that the 5/2024 LSLR Plan states that there are 4 appendices (A-D), and the 7/2024 LSLR Plan states that there are 3 appendices (A-C). Unfortunately none were included in the response.
Can you please provide me with copies of each of the 4 appendices enclosed with the May 2024 plan, and each of the 3 appendices enclosed with the July 2024 plan? Thanks very much.
Yours sincerely,
Charlie Kratovil

Dear Mr. Harley,
I am confused by the response. In addition to the Notice of Non-Compliance, the response also includes two different 11-page LSLR Plan documents, but neither of those plan documents includes the appendices that were supposedly "enclosed," according to the final page of each.
Here's what it says at the end of the May 2024 LSLR Plan:
12. APPENDIX Check all that apply and are enclosed
☒ Appendix A: Lead Service Line (LSL) Inventory Report (DEP_10-S_00014)
☒ Appendix B: Map
☒ Appendix C: Progress Report
☒ Appendix D: Customer notice of LSL Material Letters
Here's what it says at the end of the July 2024 LSLR Plan:
12. APPENDIX Check all that apply and are enclosed
☒ Appendix A: Lead Service Line (LSL) Inventory Report (DEP_10-S_00014)
☒ Appendix B: Map
☒ Appendix C: Progress Report (DEP_10-S_00027)
Were these appendices actually enclosed with the plans? If so, can you please send me the appendices referenced on the final page of each documents? If not, please confirm they were absent from the plans.
Thank you kindly.
Yours sincerely,
Charlie Kratovil

Dear Mr. Harley,
I never heard back from you regarding the appendices. I understand there is now a more recent version of the LSLR plan. Can you please provide a complete copy of it including appendices pursuant to the OPRA?
Yours sincerely,
Charlie Kratovil
