Dear Kelly Mager,
Please note the statue as referenced earlier, below:
b. Each emergency medical services dispatch center shall report to the department, in a standardized format as the commissioner shall prescribe by regulation, the following information concerning each request for emergency medical services received by the dispatch center:
(1) The date, time, and location of the request for emergency medical services;
(2) The nature and circumstances of the emergency, as provided to the dispatch center;
(3) The identity of each emergency medical services provider dispatched to the scene of the encounter; and
(4) Any other particulars of the request as may be relevant or as may be required by the commissioner.
--------------------------
Dunellen EMS does not operate an "emergency medical services dispatch center" and therefor cannot possibly maintain the above records. Somerset County Communications is the contractor of note currently providing these services to the Borough of Dunellen. These records are required to regularly be submitted to the NJ Department of Health and are therefore reasonably considered to readily accessible without need for extension.
Please kindly provide the records as requested.
Yours sincerely,
E.M. Smith

Dear Kelly Mager,
Thank you for your timely response! Unfortunately the documentation provided is incomplete and does not contain "[t]he identity of each emergency medical services provider dispatched to the scene of the encounter". Records should include dispatch records of each instance EMS is requested along with each unit dispatched, regardless of level of care.
For ease of access, I have included the template provided by the NJ Department of health below:

Thank you again,
E.M. Smith
Dear Kelly Mager,

That extension was to December 6, 2021. Therefore, the extension period has not expired and the records have been provided in accordance with the extension of November 15, 2021.
