Nationwide Insurance Complaint Records – NAIC State Based Systems (2020–Present)

Response to this request is long overdue. By law, under all circumstances, New Jersey Department of Banking and Insurance should have responded by now (details). You can complain by requesting an internal review.

Dear New Jersey Department of Banking and Insurance,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

Pursuant to the New Jersey Open Public Records Act and the common law right of access, I request copies of records maintained by the New Jersey Department of Banking and Insurance relating to a complaint submitted through the NAIC State Based Systems (SBS) consumer complaint portal from January 1, 2020 to the present.

Specifically, I request:

All responses, correspondence, or submissions provided by Nationwide Insurance regarding the complaint.

Communications between the Department and Nationwide Insurance regarding the complaint.

Any records reflecting the Department’s review, analysis, determination, or disposition of the complaint.

If responsive records exist, please provide them in electronic format. If any portion is exempt from disclosure, please provide the non-exempt portions and cite the statutory basis for any redactions or withholding.
