This is a request for government records made pursuant to the New Jersey Open Public Records Act, N.J.S.A. 47:1A-1 et seq., and the common law right of access.

Please provide copies of the following records related to a motor vehicle accident that occurred on August 30, 2024, at or near the intersection of Mantoloking Road and Adamston Road in Brick Township, Ocean County, New Jersey:

Any and all records, claim files, claim numbers, notices of claim, loss notices, or incident reports concerning the involvement of a 2020 Chevrolet Tahoe owned by the Township of North Bergen and operated by Hugo D. Cabrera.

Any correspondence, emails, letters, or other communications between the Township of North Bergen (or its representatives) and the Garden State Municipal Joint Insurance Fund (or its administrators, claims administrators, or attorneys) regarding this accident or any related claim.

Any payment authorization requests, settlement documents, release agreements, or records of payments made in connection with this claim.

Any loss runs, claim summary reports, or internal claim status reports that reference or include this specific incident.

Any documents reflecting the Fund’s determination of coverage or denial of coverage for this incident.
I request that the records be provided in electronic format (PDF) if available. If any portion of the requested records is exempt from disclosure, please release all non-exempt portions and identify the specific exemption(s) claimed for any withheld material, as required by law.
