To whom it may concern,

On or about Thursday, January 2, 2020 at approximately 10:50AM a motor vehicle accident occurred on Route 206, Flanders, NJ 07836. The victim (Frank Iaconio Jr. 34 Year Old Male) was operating a 2002 Chevrolet Cavalier and was ultimately declared deceased after being transported to Morristown Medical Center via Atlantic Air One.

I am respectfully requesting any and all Morris County Sheriff's Office (Including CIS and any other Unit/Section(s) that responded) reports including but not limited to; Investigation Report(s), Supplemental Investigation Report(s), CAD Report(s), Operations Report(s), SR-1 and 21 (R 2-04) MVC Accident Report(s) as well as any written statements from witnesses etc., regarding this event.
