Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM seeking records in connection with a legal proceeding.

Records requested: to date, I have only received two pages of the patient care report. These pages reference injuries EMS personnel observed but they do not describe the severity or possible causes of those injuries ,no timeline, no vital signs, intervention notes, or delayed assessments were provided. this is not consistent with with what would be a complete patient care report for such a critical incident, I am formally requesting a complete unredacted patient care report including all EMS notes, timelines, vital signs ,interventions, injury descriptions, and any diagrams or photographs, copies of any internal EMS incident reports, run sheets, and communications related to my son's transport and treatment, any policies or procedures governing EMS documentation and critical injury and homicide cases
