Please furnish the Certificate of Passing/Attendance of Level I Firefighting at the approved Fire Academy for Michael Reina.

Please indicate if any Certificates were gained during his tenure for the following ICS 100, ICS 200, ICS 300, ICS 400.

Please verify his length of service, and reason for departure.
