Records requested:
Referring to the Middlesex County Prosecutor's Office Policy 09-18, Evidence Procedures, I am requesting a copy from the Evidence Computer Tracking System Report showing evidence submitted to and removed from MCPO custody between February 5, 2020, through February February 12, 2020.

Additionally, I am requesting a computerized Chain of Custody Report/Status Report for all evidence under the control of the MCPO Evidence Custodian for Grand Jury Case # 20000517, Unit File # 20-00136 from receipt of said evidence to date.

MY FOUNDATION FOR MY REQUEST:
It is my understanding of MCPO Policy 09-18 that it is the responsibility of the Evidence Custodian to maintain current and accurate written/computerized records in the Evidence computerized tracking system, including records that reflect the chain of custody of property/evidence while in the possession of the MCPO. (Page 8, #5)

Furthermore, I understand that MCPO policy 09-18 requires :
• Maintaining current and accurate written/computerized records in the computerized tracking system, including records that reflect the chain of custody of property/evidence while in the possession of the MCPO. (Page 8, #5), and that
• The Evidence Custodian is responsible for Generating status reports related to property/ evidence in custody, as required. (Page 8, #14) and,
• Maintain Detailed lists of evidence maintained in the computerized tracking system. (Page 9, 15E)

Under Section "B," PROPERTY/EVIDENCE COMPUTERIZED TRACKING SYSTEM:
• A computerized tracking system is utilized to manage/track all property/evidence that comes into MCPO custody. This system serves as a single source filing system providing consistency and uniformity in the handling of property/evidence. Any inquiry about the status of property/evidence in the custody of the MCPO should begin with an inquiry through this system.:
3. The system contains the following information (minimally):
a. an identification number unique to the item;
b. description of each item, including identifiers such as make, model, and unique
identifiers such as serial number and owner applied for number;
c. the name of person(s) to whom the item is related (e.g., defendant, owner, finder,
etc.);
d. date that each item came into law enforcement custody;
e. identity of the law enforcement officer who took custody of the property
(name, initials, badge/ID number);
f. location where each item is stored;
g. date each item was entered into storage;
h. identity of the law enforcement officer who entered the item into storage;
I. All persons delivering evidence to an MCPO Evidence facility must submit an itemized
list with the evidence being delivered
