Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

Mandatory Training Certificates: Copies of all "Certificates of Completion" or records of attendance for and all current and former members of the Salem City Planning Board and Zoning Board of Adjustment who served at any time between January 1, 2023, and the present, documenting their completion of the mandatory basic course in land use law and planning required under the Municipal Land Use Law, N.J.S.A. 40:55D-1 et seq., and its implementing training provisions.

2. Municipal Compliance Report: All records or lists maintained by the DCA showing the compliance status of the City of Salem’s land-use board members for the years 2024, 2025, and 2026
3. Training Notifications: All records of notifications sent by the DCA to the City of Salem Clerk or Planning Board Secretary regarding the failure of board members to complete mandatory training during the requested period.
4. For any board member listed above or currently serving for whom the City does not possess a certificate of completion, I request any "waiver" or "exemption" documentation filed with the City or the NJ Department of Community Affairs (DCA) explaining why the mandatory training was not completed within the applicable statutory grace period.
5. Copies of any invoices, vouchers, or payment records from 2023 to the present reflecting the City of Salem's payment to the NJ Department of Community Affairs (DCA) or any other approved provider for land use training programs for board members.
6. A copy of the current official roster of the Salem City Planning Board and Zoning Board of Adjustment, including the date of appointment and term expiration for each member.
