I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records sufficient to identify the make, model, and manufacturer of the electric bicycle, electric dirt bike, or motorcycle involved in the fatal crash that occurred on or about August 26, 2026, at or near the intersection of Moorage Avenue and Sherman Avenue in Berkeley Township, New Jersey.

Please also provide any photographs of the electric bicycle, electric dirt bike, or motorcycle involved in the crash.

If available, please also provide any body-worn camera footage depicting the electric bicycle, electric dirt bike, or motorcycle involved in the crash.
