Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Requestor certification: This request is not made for a commercial purpose and is not connected to any legal proceeding. This request is made under OPRA, N.J.S.A. 47:1A-1 et seq., and, in the alternative, under the common law right of access.

Records requested — all records identified below concerning Lakewood Student Transportation Authority LLC (EIN 81-3531813) for the period January 1, 2025 to the present:

1. All correspondence, emails, letters, notices, or memoranda between the Lakewood Board of Education — including but not limited to the Business Administrator/Board Secretary, Superintendent, Board Attorney, Board President, and the State Monitor — and LSTA, its officers, agents, or counsel, concerning LSTA's federal tax-exempt status, 501(c)(3) determination, IRS filings, Form 990 filings, or corporate or organizational status.

2. All internal correspondence, emails, or memoranda among Board officials, staff, counsel, or the State Monitor concerning the revocation of LSTA's federal tax-exempt status, including any records reflecting the date the Board first became aware of the revocation.

3. Any written notice submitted by LSTA to the Board pursuant to the continuing-obligation provisions of the Statement of Ownership Disclosure (N.J.S.A. 52:25-24.2) or the Disclosure of Investment Activities certifications executed in connection with the 2025-2026 Contract Renewal Agreement, including any notice of a change to information previously certified.

4. All records reflecting any review, verification, or due diligence performed by the Board or its counsel as to LSTA's corporate form, entity type, ownership, or tax-exempt status in connection with the 2025-2026 Contract Renewal Agreement dated September 14, 2025.

5. Any correspondence between the Board and the New Jersey Department of Education, the Executive County Superintendent for Ocean County, or the State Monitor concerning LSTA's corporate or tax-exempt status.

6. Any addendum, amendment, or modification to the 2025-2026 Contract Renewal Agreement, including any addendum issued following completion of the October 15, 2025 DRTRS.

If any responsive record is withheld in whole or in part, please identify the record and state the specific legal basis for the withholding. If a privilege is asserted, please provide a privilege log identifying the author, recipient, date, and general subject matter of each withheld record.

Please provide records electronically at no charge where possible. If any special service charge is anticipated, notify me in advance and obtain my approval before proceeding.
