Dear Clark Township,

Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

Records requested:

LOSAP trust reserve

While the trust fund ledger is open: the Trust Fund's "Reserve for LOSAP"
received $265,422.94 during calendar year 2022 (per the 2022 Report of
Audit and Annual Financial Statement), of which only $60,000 corresponds to
that year's budget appropriation. I request:

1. The general-ledger transaction detail (receipts and disbursements) for
the Reserve for LOSAP for calendar years 2021 and 2022, sufficient to
identify the source of the 2022 receipts — including any remittance
advice, statement, or correspondence from the LOSAP plan administrator
accompanying a return of funds (e.g., forfeited non-vested participant
accounts), and any resolution or authorization directing the deposit.
A system-generated account activity report satisfies the ledger portion
of this item.
2. The annual LOSAP participant certifications (the certified lists or
counts of volunteers qualifying for a contribution, as submitted by the
Fire Department and Emergency Squad and certified by the Township) for
program years 2018 through 2025, together with any annual statement
or report from the LOSAP plan administrator to the Township for the same
years. Participant names may be redacted; the counts and totals are what
is sought.
