I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

accident reports, incident reports, aided reports, operations reports, internal affairs records, vehicle reports, investigation reports, citizen or victim or witness statements, citizen reports, detective bureau records, citations, summonses issued, charge sheets, disposition of charges, court records, legal actions, lawsuits filed, settlement agreements, judgements, CAD / dispatcher / police journal entry records, ambulance reports, patient care reports, fire department response reports, sobriety test results, 911 tapes, any dispatch calls, dispatch recordings, images, recordings, video recordings, surveillance video, dashboard camera video, body worn camera video, summonses issued, warrants, arrest reports, CDR-1 forms, CDR-2 forms, traffic summonses, and video of any persons being interviewed by police, referencing Jordan Clarke in the past 365 days.
My preferred delivery method for response(s) to this request is by E-mail as attachments. Please confirm you have received this request. If you are not the custodian of records, please forward my request to that person and provide their email address to me for future reference.

The Borough of Paramus is in receipt of your OPRA request. Please note in order to receive an EMS/Ambulance report, the release papers from the patient stating it can be released to you are required. If you would like a copy, please fill out a new OPRA request with the proper documents. The other parts of this request have been sent to the proper Departments for processing.
