items prior to a request being processed:

Under penalty of N.J.S.A. 2C:28-3, I certify that:

1. I HAVE / HAVE NOT (select one) been convicted of any indictable offense
under the laws of New Jersey, any other state, or the United States; 

2. I, or another person, WILL / WILL NOT (select one) use the requested
government records for a commercial purpose; and

3. I AM / AM NOT (select one) seeking records in connection with a legal
proceeding.

Kindly submit your certifications or a completed OPRA form to this Office
by close of business on December 5, 2024 so that we may proceed
accordingly, or your request will be considered withdrawn and closed.
