1. The date and time which this investigation was initiated by Chief Mark Tinitigan and then assigned to Acting Lt. Peter Roselli to conduct an investigation against Sgt. Minacapelli and Ptl. Guiamano in the year 2021. Please include any notes, any emails sent or received, any supporting documents related to, which resulted in a subsequent charge of New Jersey Title 40a:14-147, charge of "incapacity "against both officers on or about November 15, 2021 .

2. The date and time when this investigation was completed by the assigned officer, Acting Lt. Roselli and also the date and time which this matter was then handed over to the law enforcement executive, Chief Mark Tinitigan.

3. Please provide the date and time which this investigation was then provided to Councilman James Gurchensky and the public safety committee.

4. Please include a copy of every report, all notes, emails sent or received and any related supporting documents given to or provided by Acting Lt. Peter Roselli.

5. Please provide all completed reports, notes, all emails and supporting documents received or sent by Councilman James Gurchensky or any of the public safety committee members.

6. Please provide all completed reports, all notes, all emails and supporting documents received or sent by any other Borough of South River employee regarding this investigation of Incapacity against Sgt. Minacapelli and Ptl. Guiamano

7. Starting From May 28, 2021 to present, Please include any reports, any notes, any emails sent or received or any supporting documents given to or provided by Chief Mark Tinitigan regarding this investigation that he initiated against Sgt. Minacapelli and Ptl. Guiamano.

8. Please also include any letters, all emails , all correspondences sent or received from any Borough of South River employee involved in this process during this investigation against Sgt. Minacapelli and Ptl. Guiamano.

9. Please include any emails sent or received, any replies by Chief Mark Tinitigan regarding inquiries or requests to obtain a copy of this investigation report regarding Sgt. Minacapelli and Ptl. Guiamano.

10. Please include all copies of notes taken by any other employee involved in any meetings, conversations, including the members of the public safety committee regarding the investigation and charges of Sgt. Minacapelli and Ptl. Guiamano.

11. Please provide a complete Law Soft system data audit print out of all said reports and the Law Soft system print out of each time stamp alteration of each report(s) completed by any officer involved in this investigation regarding Sgt. Minacapelli and Ptl. Guiamano in the year 2021.

12. Please provide all list of everyone present, their locations, dates, and times, when the matters of Sgt. Minacapelli and Ptl. Guiamano's charges of incapacity and notices of termination were discussed in 2021 by any Borough of South River employee or the Mayor and Council persons.

13. Please include a copy of any prior training, any training certificates that councilman James Gurchensky has received in handling any Police department investigation matters, his training and knowledge of New Jersey Laws pertaining to Law enforcement departmental investigations, his prior training with handling New Jersey Title 40a law.
