Please accept this electronic request for public records made under OPRA and the common law right of access. Records requested:
There was an incident that took place on Thursday, July 23 involving my son Leon Mayo and his employer A.J. Images — located at 259 E. 1st Ave., Roselle, New Jersey.

Police responded, my son was taken to the hospital for evaluation and released. The incident resulted in my son being terminated.

I would like a copy of the incident report for his file In an attempt to see how it may have an impact on his future employment prospects.
