I am requesting all records on 570 Lee St, Perth Amboy.
This is a request for public records made under OPRA and the common law right of access. I am not required to fill out an official form. Please acknowledge receipt of this message.
Records requested:
I am requesting all records on 570 Lee St, Perth Amboy.
1. Results of any studies or surveys of HVAC systems in the building for the following period January 2, 2023, until the present.
2. All purchase orders for HVAC maintenance or installations since January 1, 2022.
3. Any and all logs and reports of completed HVAC maintenance work by district employees or outside contractors since January 1, 2022.
4. Any purchase orders for engineering controls for COVID-19, such as plexiglass barriers, ventilation systems, air sanitizers, or any other equipment since August 1, 2020. Installation/maintenance records for same if the equipment has been delivered and installed.

2. Please see attached PDF. Current amended contract $5,691,988.00
3. All work performed by the contractor is attached in the PDF (Otlowski
HVAC work Status)
4. No Air Sanitizers where installed. Project was designed prior to COVID
19. County Facilities followed any COVID protocols.
