I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

Copies of all formal logs, tracking sheets, summaries, reports, written complaints, interviews or executive session minutes (redacted* as required by law) created, maintained, or processed by the Affirmative Action Officer, Title IX Coordinator, Board Secretary or superintendent between January 1, 2026, and the present date, regarding reports, allegations, or investigations of workplace harassment or hostile work environments.

*Redaction in Lieu of Denial: If any responsive record contains confidential personal identifiable information (PII) or exempt personnel details, please redact the exempt portions and provide access to the remaining disclosable portions of the record, pursuant to N.J.S.A. 47:1A-5(g).
