Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-6(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:

I am requesting access to any and all public records maintained by the Mantua Police Department relating to Horace J Johnson iii DOB 12/07/1983, including but not limited to:
• Any criminal charges, arrests, summonses, or complaints (filed, pending, dismissed, or adjudicated);
• Any video footage, body camera footage (highly requested) incident reports, investigative reports, or police narratives in which is named Horace Johnson as a suspect, defendant, or arrestee;
• Any case numbers, complaint-summonses, or arrest records associated with the above;
• Any court disposition information reflected in police records.

This request includes records from any date range maintained by the Police Department ranging from 2024-2026. If any portion of the requested records is exempt from disclosure, please provide all reasonably segregable, non-exempt portions in accordance with OPRA.

If no responsive records exist, please provide a written certification to that effect.
