Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

I AM NOT seeking records in connection with a legal proceeding.

Records requested:
Pursuant to N.J.S.A. 47:1A-1 et seq., I am requesting the following government records maintained by the City of Pleasantville:

1- All Fire Department incident response logs, run sheets, and National Fire Incident Reporting System (NFIRS) reports for a structural fire at 127 W. Thompson Avenue occurring between January 1, 1990, and December 31, 1999.

2- All building, demolition, and construction permits issued for 127 W. Thompson Avenue following said fire through December 31, 1999, to establish the timeline of structural reconstruction.

3- The local registrar vital statistic death record or certificate index entry for decedent George Clayton, who passed away during the 1990s at this address, to confirm the exact date of the incident.
