Please accept this electronic request for public records made under OPRA and the common law right of access. I am not required to fill out an official form or use a particular software platform to submit my request per NJSA 47:1A-5(f), which states that an email from a requestor including all of the information required on the adopted form shall suffice in place of a completed form as a valid government record request.

I HAVE NOT been convicted of any indictable offense under the laws of New Jersey, any other state, or the United States.

I WILL NOT use the requested government records for a commercial purpose.

Specifically, I am requesting the following:
1. All fire incident response records for this address, including dispatch reports, incident summaries, and response narratives.
2. Any hazardous materials (hazmat) incident records, including chemical releases, spills, or related emergency responses.
3. Any Fire Marshal investigation reports, if applicable.
4. All fire code violations, Notices of Violation (NOV), or enforcement actions issued to this property.
5. Any records of flammable or combustible liquid storage, including underground or aboveground storage tank permits or registrations.
6. Any permits, applications, renewals, or approvals associated with the property.
7. Any historical records-maintained pertaining to fire safety, hazardous materials, or emergency response at this location.
